MPKS withdraws Google form for Covid-19 vaccination at Unimas PPV effective immediately after reports of misuse

KUCHING, June 20: The Kota Samarahan Municipal Council (MPKS) has today withdrawn the Google form used to register Covid-19 recipients for vaccination at the Universiti Malaysia Sarawak (Unimas) vaccination centre (PPV).

Its chairman Dato Peter Minos said the form has been misused by some quarters with the intention to confuse the public although the form was initially created by the council on June 17 to be filled by Samarahan residents to register for vaccination and to be submitted to Unimas for further action.


“MPKS has now discovered that its form had been improperly used, probably with intention to confuse the public.

“Therefore from today (Sunday, June 20), the MPKS form mentioned above is immediately withdrawn, closed and no longer in use,” he said in a statement.

Minos said the council is not amused and not pleased by those frustrating MPKS’ good intention to help out with the government’s Covid-19 vaccination exercise.

On another note, he disclosed that there were 68,423 forms completed and received by MPKS today.

He added MPKS is screening and processing the forms and data therein before submitting them to Unimas for further action. — DayakDaily
